FAA psychological and Neuropsychological evaluations

- Please provide us with a copy of the FAA letter requesting testing

- Request that the FAA send a copy of your medical record directly to the Neuropsychologist (see the "Forms and Contracts" section of this website for a copy of the request form pre-filled with Dr. Edwards' contact information)

- Provide any additional documentation or records prior to the appointment
